The inaugural international guidelines for the diagnosis of rheumatic heart disease (RHD), a disease that affects tens of millions of people worldwide, have today been published by the World Heart Federation in Nature Reviews Cardiology.
The guidelines define the minimum requirements needed to diagnose RHD in individuals without a clear history of acute rheumatic fever (ARF), and will have important global and national implications.
Diagnosis is conducted with an ultrasound of the heart's valves and chambers, known as an echocardiogram, but currently no guidelines are available to define what is normal on echocardiography.

To treat type 2 diabetes, we typically use lifestyle, and that may work alone -- just diet and exercise -- then we may need oral medications, and it is not uncommon for someone with type 2 diabetes to eventually need insulin, either with or without the oral medications.
Now, type 2 diabetes accounts for approximately 85 to 90 percent of all the diabetics in the country. The other important thing that needs to be said is that type 2 diabetes is associated with heart disease, and that's why it's so important to not only treat the glucose levels, but also to attack blood pressure and cholesterol levels as well. We know that type 2 diabetes runs very strongly from generation to generation, and we also know that we can prevent type 2 diabetes if we recognize it at an early stage.
The last important thing to mention about type 1 and type 2 diabetes is that you can get any type of diabetes at any age. That's very important and that's why the American Diabetes Association changed the name from 'juvenile onset' to 'type 1,' and 'adult onset' to 'type 2.'

The key word in creative destruction, however, is "creative." Now we're living in another time not unlike the 1930s, with a jobs crisis that's partly a massive failure of financial markets and partly a huge technological shift in the nature of work. There's no question the Great Recession slammed the global economy. But one reason why the jobs market has been so slow to recover is that technology is enabling us to do more work with fewer people -- or with people anywhere around the world.
Ah, but your grandfather was a blue-collar worker, you may say. Those kinds of jobs are begging to be automated. If he'd gone to college, that would have been a different story.
And that's very true: if my grandfather had gone to college he probably wouldn't have been an ice man, or a farmer. But an education isn't the guaranteed haven from technological change it used to be. The working assumption that most people have -- that technology favors the smart, the creative, and the well-educated -- may not hold up any more.
Figure it this way: it's about the difference between repetitive tasks and those that require analysis. If you're working on an assembly line, picking vegetables or handling deposits and withdrawals over a bank counter, a machine might do your job better. If you're in charge of making sure those jobs get done, or marketing them, then a computer may help you, but it can't do the job for you.
Unfortunately, the definition of "repetitive" is going to keep shifting. "E-discovery" software, which can sort through email and documents looking for suspicious patterns, is already taking on a job traditionally done by paralegals and junior associates in law forms. IBM's "Watson" computer, which can respond to questions well enough to play "Jeopardy," is really designed to take over tasks from nurses and doctors, like taking medical histories. But you'll still need a human being to write a brief, argue in court, or conduct your surgery.
The jobs crisis is the first priority for most Americans, and rightly so. If you don't have a decent job in America, your entire life can unravel. Yet in the early stages of a crucial presidential campaign, we're spending far too much time asking the wrong questions: can we "hold onto" the jobs we have? Should we cut taxes? Does a college education pay off?
What we really need to do -- and what our political candidates better start doing -- is talk about what kind of jobs technology is likely to create, and what kind it destroys, and how our national policy can get ahead of that curve. The economy will work these issues out in the long run, but it'll be a lot less ugly if we actually start planning for the changes we know are coming. Anything else is like refusing to eat the ice cream from the refrigerator: a stand that doesn't change a thing.

This addiction to a fantasy world or alternate reality may be a result of unhappiness, chronic negativity, lack of self-esteem or inability to have a real world relationship.
Talking online allows that person to feel desired, sexy and connected, which in turn releases those ?feel good? chemicals in the brain. The more that online interactions occur the more the brain wants that reward again, causing the cycle to continue and resulting in withdrawal from the real world and reliance on the virtual world found online.

The most promising candidate on the lineup is Cabbies Star, Droopys Scolari?s three-year-old black daughter out of bitch Final Orchid. She was third in her first two starts of the new season but then bucked up towards the third race of 2012.
She was second in the Grade A3 William Hill Supporting Racing Post TV Stakes on 8th February at Sunderland. Cabbies Star was one of the three 5/2 collective favourites of the race. She finished racing over the 450 metres track in 27.88 seconds
and missed the winner?s position by an unfortunate shoulder?s distance.
The other two collective favourites, Fois Savings was first while North Uist was fifth behind the winner.
Cabbies Star posted her seasonal debut victory on 17th February when she won the Grade A3 William Hill ? The Home Of Betting Stakes at Sunderland. The 6/4 favourite of the race went through the wire in 27.64 seconds, leading by 1 ? lengths. She
was followed by Boutros and Seaside Mary Ann.
Today?s race gives her the opportunity of recording her second victory of the season.
Another potential candidate on the field is Witton Master, Droopys Kewell?s three-year-old white and black son out of bitch Witton Chloe. He has raced during the new season four times, out of which he has won twice and had been positioned fourth twice too.
Witton Master won on 5th January in a 450 metres race that he raced through in 27.97 seconds with two lengths? lead. Second after the winner was Express Concorde. On 9th February, it was during his third attempt that he struck a second
victory of the season.
He won the 261 metres race at Sunderland and was on the other side of the wire in 15.94 seconds, leading by three and a half lengths. Finishing the race closest to the winner was Derrane Lewis.

It?s important to have a built-in security belief that you are a valuable person. Unless you have this capacity when anything goes wrong in your life, you may have difficulty believing that you have any worth it all.
Be aware of your process of untangling yourself from external ?ratings? of your value as a person and this is done through unconditional self-acceptance. By applying these techniques you will not rate yourself based upon any inadequacies that you perceive, nor any feelings of failure or disapproval and you will be less likely to have a negative self-image.
In order to create self-acceptance the following statements should be followed:
1) You are a unique talented individual and human being.
2) You are constantly developing new skills and new healthy ways of coping.
3) You might be able to measure some of your externals such as how quickly you can accomplish a task at work but you can never rate yourself as an entirety because we as human beings are far too complex and we continue to modify ourselves through life experience daily.
4) It is extraordinarily important to understand that we as human beings are by our very nature not perfect and are capable of making mistakes. Anyone who believes otherwise is not being realistic and has self-goals for behavior that cannot ever be reached in real life. Nobody is perfect so stop holding perfection out as the model that you seek.
Can you see how much easier your day-to-day existence will be if you can see that you have worth just as a person and independent of how other people appear to perceive you? Everybody likes to feel accepted and admired but if you have self- acceptance you will not live in dread of losing the admiration of others.
Self-acceptance allows you to accept flaws in others and also in yourself and can lead to healthy accommodations emotionally to negative experiences. With this realistic and supportive type of emotional response to adverse circumstances you are truly adapting to your environment. If you tend to put yourself down and blame yourself for the creation of negative events in your life, this will lead to inappropriate self-valuations which in turn may well contribute to unhelpful or destructive behaviors.
If you will look inward and explore your inner self, you will be able to make solid inroads toward understanding the bases for your low self-esteem, improve your understanding of yourself intrinsically as a person of worth, and thereby adapt to negative circumstances and develop healthier behaviors in response to them. As your behaviors become healthier your confidence builds and you realize that negative events need not be blamed upon yourself. Rather, you can take pleasure in seeing how personality growth has helped you to overcome the difficulties of life and not allow them to deprive you of the day-to-day pleasures that the capacity for self-acceptance brings.
